They make another one that is a full-scale 5-string bass on one side and a short-scale 4-string fretless bass on the other side.

You are absolutely correct, there is no getting your hand behind this neck on the longer side. Given this video of their original concept for the neck, I'm guessing the designer has no thumbs.
