It is part number PT-5000-00. Fit pretty much perfectly. String spacing and height required no modification for me. Just had to file a slight bit off of the ends as it was slightly longer than the stock nut.

I'm not usually a fan of swirls at all, but I love the colours and the pattern in this one. Really nice!
