Something tells me that "Fraudulent Tax Refund Disbursements" isn't going to be a line item in the Federal Budget next year, but it probably should be - since this guy got nailed, I wonder how many millions of dollars have gone to people who haven't been caught?

Not surprising. HMRC (Her Majesty's Revenue and Customs - what was the Inland Revenue and HM Customs & Excise until they merged two years ago) sacks and prosecutes a few employees every year for things like surfing taxpayer accounts to look up celebrities or friends/family, generating fraudulent tax rebates and suchlike.

To answer eaeolian's point there are undoubtedly millions paid out incorrectly, either due to error or deliberate fraud.

I don't know how the IRS works, but quite often in the private sector and in the UK public sector a lot of debt recovery or recovery of overpayments is risk-based - so, for example, there may be certain occupations that would be known to be more open to abuse of the tax system, whether due to frequent cash payments or whatever, so that's where you'd concentrate your audits and recovery efforts.
